WIC is the Special Supplemental Nutrition Program for Women, Infants, and Children. Pregnant women, postpartum women, breastfeeding women, infants, and children under the age of 5 can receive WIC benefits.  

To qualify for Iowa WIC, you must:  
  1. Meet income guidelines - Scroll down to WIC Income Guidelines
  2. Be a resident of Iowa
  3. Have a health or nutritional need  
Types of WIC Appointments

Done every year. 

What to Bring: 

  • Yourself & all infants and/or children applying for WIC 
  • Recent hemoglobin results, weight, and height (if available) 
  • Immunization record for children 2 years old & younger 
  • Proof of Income (one of the following): 
    • Paycheck stubs (from the last 30 days) 
    • Income tax return 
    • Medicaid card 
    • Notice of award letter for Food Assistance or FIP  
  • Proof of Address (one of the following):   
    • Utility bill 
    • Rent or mortgage receipt 
    • Driver's license with current address 
    • Voter registration card with current address  
  • Proof of Identity (one for each person applying for WIC):  
    • Driver's license or passport 
    • Birth certificate
    • WIC card
    • Insurance or Medicaid card
    • Hospital/medical record
    • Crib card
    • WIC ID folder (from any state) 
    • Social Security Card
