Siouxland District Health Department's Health Maintenance Program provides services to individuals who live with a stable chronic condition.
Is the Health Maintenance Program for you?
You may qualify if you:
- Have a stable, chronic condition
- Require nursing visits 1-2 times a month
- Meet financial guidelines and follow a sliding fee scale
- Do not require intense skilled home health services
- Live in Woodbury County
Health Maintenance Services
- Medication management to include instruction and med set up as needed
- Administration of injections and blood draws
- Teaching and nursing interventions
- Maintain stability of chronic conditions
- Referral to other community agencies as needed
- Coordinate with physicians, pharmacies, caregivers, and other community agencies
- Physical, emotional, and home safety assessments
