Siouxland District Health Department's Health Maintenance Program provides services to individuals who live with a stable chronic condition. You may qualify if you:

  • Have a stable, chronic condition
  • Require nursing visits 1-2 times a month
  • Meet financial guidelines and follow a sliding fee scale
  • Do not require intense skilled home health services
  • Live in Woodbury County

Health Maintenance Services

  • Medication management to include instruction and med set up as needed
  • Administration of injections and blood draws
  • Teaching and nursing interventions
  • Maintain stability of chronic conditions
  • Referral to other community agencies as needed
  • Coordinate with physicians, pharmacies, caregivers, and other community agencies
  • Physical, emotional, and home safety assessments
