Hawki offers free or low-cost health care coverage for kids and teens up to age 18.

Eligibility requirements
- Be under age 19
- A resident of the state of Iowa
- Have no other health insurance (you can have dental insurance to qualify)
- Be a U.S. citizens or have a qualifying immigration status
- Not the dependent of a State of Iowa employee
Coverage
- Doctor and well child visits
- Specialist
- Vision, dental and hearing exams
- Vaccines (immunizations) and prescriptions
- Ambulance services and emergency care
- Inpatient and outpatient hospital services
- Surgery
- Mental health services/substance abuse care
- Telehealth

- Print, fill out, and mail the Application (Form 470-5170) Español (Form 470-5170S)

Hawki Dental Only
Hawki has a special program called Dental Only. It helps kids who already have health insurance but do not have dental insurance.
Who can get it?
A child can get Hawki Dental Only if:

They meet the regular Hawki rules (like age and family income).

They already have health insurance.

Their health insurance does not include dental care.
What does it cover?
The dental benefits are the same as regular Hawki dental benefits. This includes things like:

Checkups and cleanings

X‑rays
-
Fillings
- Medically necessary braces (prior approval is required)
- Other dental care kids need
