IMMUNIZATIONS

Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

Effective, August 1, 2016 there is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

hawk-i is a program that provides health care insurance for Iowa children in families with limited incomes. Once your child is enrolled, they are eligible for 12 months. Renewal forms must be completed and returned yearly to remain eligible.

                        Healthy kids are happy kids!

 

Children and teens under 19 years of age are eligible if they:

  • Meet income guidelines
  • Are Iowa residents
  • Are U.S. citizens or permanent legal residents
  • Are not the dependent of a State of Iowa employee
  • Are not eligible for Medicaid benefits

   

Services that are covered under hawk-i include:

 Dental Care  Vision Exams  Hearing Services

The hawk-i program offers a dental only coverage option for children who have health insurance but may not have dental insurance coverage. These services include:

Cleanings & checkups Fluoride treatments
Sealants Fillings and crowns
