Effective July 1, 2011, we will be charging a $10.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash or check only for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Water Testing, private wells PDF Print E-mail

The laboratory at Siouxland District Health Department is certified by the Iowa Department of Natural Resources (IDNR) to test water of many types, including private well water.

Available tests include coliform bacteria, nitrates, fluoride, iron, hardness, manganese, and sulfates.

The form along with a price list and collection instructions can be downloaded from the Form section of this website.

The IDNR does have a grant program in which well owners can have their wells tested for free and it applies to residents in all counties in Iowa.  Call the laboratory at 712-234-3908 or 1-800-587-3005 to get details on this program or ask other questions regarding water testing.
