Effective July 1, 2011, we will be charging a $10.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics. We accept cash or check only for payment at this time. In addition, we are unable to accept children with Hawk-I coverage. These children should be taken to their physician for immunizations.

Water sampling instructions for people collecting water for bacteria and/or nitrate testing.
This document contains a list of many common chemicals found in drinking water along with appropriate levels and potential health effects.
Use this form when submitting swimming pool samples for testing.
This form includes a test list along with price information and is intended to be for water samples that are not reportable to the DNR, such as private well water.
This form needs to be filled out in order to use the DNR grant that pays for the water testing and collection. A county sanitarian also needs to collect the sample in order to use this grant. Feel free to call SDHD to set up a collection. |
